Vivien Ziwocha

Vivien Ziwocha graduated from the University of Greenwich with an Honours degree in Health Management. She is a Registered Mental Health Nurse with experience of working with older adults, learning disabilities and complex mental health rehabilitation. In her most recent regional role, she took a lead role in developing the Dementia Voice Nurse Service nationally for Housing and Care 21. Her management experience spans from hospital management, nursing care to housing and domiciliary care. This augmented her passion to develop integrated care services for people living with Dementia.

Lisa Bradbury

Lisa is a registered mental health nurse with over 20 years experience working with adults and older adults and has just completed an MSc in Clinical Leadership at Kingston University. She has extensive experience both as a clinician and senior manager within inpatient and community settings including memory services. Lisa is passionate about delivering a high quality and effective service to people and carers living with dementia.

Lisa is able to provide:

  • Holistic thorough nursing assessments and person centred care plan development
  • Practical advice and support to promote independence and wellbeing
  • Extensive nursing assessments such as Health Needs Assessments and assessments for NHS funded care
  • Post diagnostic counselling
  • Individual Cognitive Stimulation sessions
  • Monitoring of  health in order to anticipate and act quickly to prevent deterioration in psychological wellbeing and physical health

Colette Griffin-Chapman

Colette Griffin-Chapman graduated from the University of Surrey with a degree in mental health nursing. As a registered mental health nurse she has experience in working with adults, learning disabilities and older adults. She is specialised in dementia care and recently took the lead on completing a protocol and training package for a Dementia therapy programme. Her experience spans from inpatient services to community rehabilitation and most recently community assessment and treatment for people living with Dementia. She is passionate about delivering a high standard of nursing care to promote wellbeing throughout the dementia journey.
